Application deadline is June 1st
The American Heart Association is celebrating individuals who have made a positive impact on their health and quality of life with a Lifestyle Change Award, sponsored by Consiliarium Group.
Applications for the award are currently being accepted and can be submitted by friends, family, and co-workers. People are welcome to nominate themselves, as well. A panel of volunteer judges from Consiliarium Group and the American Heart Association will review applications and determine the finalists and award-winner for the deserving individual who has made significant strides to become healthier. Whether they have lost weight, improved eating habits, lowered their cholesterol through exercise and diet, or a combination of these things and more, the Lifestyle Change Award recipient is chosen to inspire others to improve their health. No change is too small, and every accomplishment is significant. The submission deadline is June 1, 2020.
